Information regarding EmuNation 4.1.0.6

Workaround hack for MyTheater (older versions) and Agarwals MDAPI filter apps
such as MediaPortal problem which was crashing EmuNation on some channel changes.
Minor fix for potential problem switching from unscrambled to scrambled channels.
Fix to insure that the proper caid is always being used. Applies to all
systems but was required for cardsharing to work properly.
